We created the Symptom Tracker to help you keep track of your symptoms. The Symptom Tracker is simple, customizable, and does not require any apps. Since it uses Google Sheets combined with Google Forms – it’s an accessible and readable tracker that anyone can use!
Here's how it works:
Make entries about your symptoms through Google Forms.
Discover insights about your symptoms through data points and graphs in Google Sheets.
Review any notes (based on filtered criteria) in Google Sheets.
What Now?
You can use this tool to help you translate what you're experiencing as symptoms into doctor-speak so that you can say things quickly and efficiently when you talk to a doctor.
Click the button below to get access to the Symptom Tracker. Then make a copy of the Symptom Tracker Template Google Sheet to your Google Drive account. A form will be automatically generated in your Google Drive that will be linked to your Symptom Tracker.
I don't see the Google form. I can see the Symptom Tracker, but not the form to use to add entries. I see it on the video, but where is it for me to use?
